Berberine vs metformin for PCOS

One is a plant compound you can buy today. One is a prescription with decades behind it. On the numbers they look close. One detail decides between them.

For PCOS (now officially PMOS), berberine performs comparably to metformin on insulin sensitivity, glucose, lipids, and BMI, and you can buy it over the counter. But it is less studied and is not considered safe in pregnancy, so if you are trying to conceive, metformin is the option to discuss with your doctor.

Nature's metformin

Berberine is a plant compound often called nature's metformin because it activates the same AMPK pathway. A network meta-analysis found it performs comparably to metformin on insulin sensitivity, glucose, lipids, and BMI.[1] It is available over the counter, at a typical dose around 500mg two to three times a day, and it can cause GI upset, so many people build up slowly.

What metformin brings

Metformin is a prescription drug with decades of use behind it. The 2023 guideline recommends it, especially when BMI is raised.[2] That long track record is the main thing it has over berberine. It is the more proven option, and its safety profile is far better mapped.

The detail that decides it

Berberine is promising and buyable today, but it is less studied than metformin, and importantly it is not considered safe in pregnancy. So it is not for anyone trying to conceive. Metformin is the more proven option and can be used while trying to conceive under a doctor. That single point does most of the choosing for you.

Neither is the whole answer

Both of these lower insulin, and so does the rest of your life. Neither is a substitute for the food, movement, and sleep that lower insulin too.[2] A supplement or a prescription works best on top of those, not instead of them.
