Can a phone camera really read your heart rate?
Short answer: yes, and it is a real, peer-reviewed technique called rPPG. Longer answer: it depends heavily on how you take the reading, and there is plenty it cannot do.

Face-scan apps use remote photoplethysmography (rPPG). Each heartbeat changes how much light your skin absorbs, and the camera picks up that rhythm. Validated against ECG, camera-based pulse measurement lands within about 1 beat per minute in good conditions. It measures pulse and related signals. It does not diagnose disease, and bad lighting or movement will wreck the reading.
How it works
Every time your heart beats, it pushes blood into the vessels just under your skin. That changes how much light your face absorbs, very slightly, in a rhythm that matches your pulse. The change is far too subtle for you to see, but a camera can detect it across many frames and many pixels. Software isolates that signal from everything else in the frame and reconstructs the beat pattern. This is remote photoplethysmography, and it is the contactless cousin of the clip the nurse puts on your fingertip.
How accurate it actually is
Better than most people expect, with caveats. A clinical validation in cardiovascular disease patients found camera-based pulse measurement agreed with ECG to a mean absolute error of about 1.06 beats per minute, with a correlation of 0.96.[1] A meta-analysis of consumer-grade contactless monitors reached a similar conclusion for heart rate against medical devices.[2] That is for pulse rate specifically, which is the signal rPPG does best.
What ruins a reading
The honest weak spot. Accuracy depends on conditions, and reviews of the field consistently flag lighting and motion as the main failure modes.[3] Practically: face a light source rather than sitting with a window behind you, aim for bright and even light, keep still, and let the scan run at a decent frame rate. Forehead and cheeks carry the strongest signal, which is why these apps want your whole face in frame.[4] Take it the same way each morning and the trend gets far more trustworthy.
What it cannot do
A face scan does not diagnose anything. It estimates vital signs. It cannot detect heart disease, screen for illness, or replace blood work or a clinician, and any app telling you otherwise has walked out of the general-wellness lane and into regulated medical-device territory.[5] What it can do is give you an objective, repeatable number that you take yourself, for free, in thirty seconds, on a device you already own. That is the whole pitch, and it is enough.
